Case Management

SNAP Applications and Recertifications

The Supplemental Nutrition Assistance Program (SNAP), formerly known as Food Stamps, provides food assistance for nearly 1.8 million low-income New Yorkers. We assist individuals and families in our community with applying and securing benefits from this much-needed program.

In addition to SNAP applications, we also provide recertification services for SNAP by appointment only. We are currently seeing SNAP clients from 8am to 2:30pm Monday through Thursday. Care for the Homeless

We work with Care for the Homeless, Inc., to keep our guests healthy through essential medical services. The Care for the Homeless Clinic is currently open Monday-Thursday from 9am to 1pm. For more information please call 718-574-0058 x111.

The Care for the Homeless team:

  • Provides COVID-19 vaccines and testing
  • Assesses immediate and chronic health issues, such as asthma, diabetes, hypertension, bronchitis and pneumonia
  • Connects guests with appropriate services based on individual health needs
  • Provides rapid HIV and T.B. testing
  • Offers intensive case management as needed

The casework team at Bread & Life is dedicated to connecting guests with resources and benefits including but not limited to:

  • Medicare
  • Medicaid
  • Social Security
  • Temporary Assistance for Needy Families (TANF)
  • Rental Assistance
  • Referrals to Urban Justice
  • ERAP Application Support
  • Mail Service (8am to 3:30pm Monday through Thursday)
